  2. Schedule a meeting, fill out and send the registration form, email or in person
  3. Visit Center: The Manger will review the registration form and create a needs assessment
  4. A weekly schedule is arranged
  5. Service becomes available

In-Home

  1. Contact S.R.M to get the information kit and costs
  2. Schedule a meeting, fill out and send the registration form by email or in person
  3. Home Visit: The S.R.M team member will review the registration form and will observe the needs
  4. A weekly schedule is arranged
  5. Service becomes available

Residences

  1. Contact S.R.M to get the information kit and costs
  2. Residence Visit: the residence owner and S.R.M team member will go over the registration form and create an activity plan
  3. A weekly schedule is arranged
  4. Service becomes available
